Our Projects

 As a PCN we co-ordinate with the following projects:

  • We have a Mental Health worker within the ARRS funding but we also have GP aligned CPNs which can assess patients with complex mental health needs and help GPs manage them safely.
  • The Community Matron Team work within the Care Homes in Derwentside- they respond to new patient health concerns reported by staff and aim to do a weekly ward round to assess patients in need of review. They can assess patients and formulate diagnoses but are unable to prescribe. They will discuss the weekly clinical queries list with the allocated doctor at the responsible practice to resolve the Care Home staff questions.
  • Enhanced Care within Care Homes is part of the NHS Long Term Plan which started in 2020. This is a 4-year plan

Training and Education

As a PCN we offer high quality training and education to our member Practices and staff on an ongoing basis. Learners include a range of people, from pre-registration Pharmacists to Practice Managers. The PCN has two Educational Leads- Dr Victoria Cliff for clinical areas and Gillian Webster for non-clinical areas. As Educational Leads, Victoria and Gillian advocate for training and education at PCN level. They are able to update the PCN on educational developments and develop shared training whilst networking to share good practice and supporting learners, as well as recieving and building based upon feedback from learners themselves. The aim is to increase awareness and participation regarding training. This subsequently improves job satisfaction and professional development for staff which leads to improved staff retention across the PCN, and crucially improves care standards for the public.
